Restated Articles Form Instructions - Nonprofit

The instructions are listed in numerical order to align with the form. These instructions are intended for use with restating the original articles for nonprofit corporations. They supplement the instructions on the form itself.
  1. NAME OF THE CORPORATION:  Complete the existing name of the nonprofit corporation.

  2. NEW NAME OF CORPORATION (if changed): This should only be completed if the nonprofit's name is changing.

  3. A COPY OF THE RESTATED ARTICLES MUST BE ATTACHED:  It is required that the articles being restated are attached as a separate document.

  4. CHECK THE APPROPRIATE STATEMENT:  How were the Restated Articles approved?  Choose one of the two options, and complete the information as required and applicable.

  5. EXECUTION:  This legal declaration must be signed. If another form is generated, this exact wording must be used. ​​​
