Assumed Business Name Amendment Form Instructions

The instructions are listed in numerical order to align with the form. They supplement the instructions on the form itself.
You only need to complete the sections you are updating, but fully complete them.

REGISTRY NUMBER:  Must be entered.

  1. CURRENT BUSINESS NAME:  Enter the current Assumed Business Name, even if it is not being changed.  DO NOT enter the name of an LLC or Corporation that owns and uses the name.

  2. NEW BUSINESS NAME:  Enter the new name. Do not enter the name of an LLC or Corporation that is using the name.
    NOTE: The registration of a name does not grant exclusive rights to that name. A name may be available for registration, but another person may have a prior legal right to the name or a very similar one. If the name is similar to another well-known name, it could be a cause for legal action.

  3. DESCRIPTION OF BUSINESS: A brief description of the business activity.

  4. PRINCIPAL PLACE OF BUSINESS:  Physical address where the business is located. This address may also be where the business records are kept.  The principal office may not be a PO Box, a commercial mail receiving agency, a mail forwarding business, or a virtual office.

  5. NAME OF AUTHORIZED REPRESENTATIVE:  The person or business who is authorized to represent the owner(s), and will receive mailed notices.  For a continuing Authorized Representative who has had a name change, mark “NEW”. 

  6. MAILING ADDRESS OF AUTHORIZED REPRESENTATIVE:  An address where renewal notices will be mailed. Other types of mail will also be delivered to this publicly available address.

  7. REGISTRANTS/OWNERS:  This section is used to update for new or additional owner(s).

    a. NEW REGISTRANTS/OWNERS:  The name of the new individual or business entity who will own and do business as the Assumed Business Name. The physical address may not be a PO Box, a commercial mail receiving agency, a mail forwarding business, or a virtual office.  If more than one, use a separate sheet.

    b. WITHDRAWING REGISTRANTS/OWNERS:  The name(s) of existing owner(s) who will no longer do business as the Assumed Business Name.  No address required.

  8. CURRENT REGISTRANTS/OWNERS ADDRESS CHANGE ONLY:  Use this section only to update an existing owner’s street address.  It may not be a PO Box, a commercial mail receiving agency, a mail forwarding business, or a virtual office.

  9. COUNTIES:  Select the counties in which the name should be registered.  This will keep others from registering the same name in those Oregon counties.

  10. SIGNATURE(S): New Registrants must sign. If any Registrants are withdrawing, the withdrawing Registrants or Authorized Representative must also sign.​​​​
